[cisco-voip] Unity 4.0(4) PCA - User Page

Somehow I have messed up my access to the Unity PCA page.  I can login
to my Unity account (http://Unity/ciscopca).  

When I click Cisco Unity Assistant I am directed to the Front page with
the Recorded Name control bar.  My browser acts like it is trying to
install AvMediaMasterCtrl applet.  My browser hangs and I never get a
response.

It seems like I just need to re-install the applet, but anything I try
doesn't work.
