[cisco-voip] pri & mgcp

Im trying to configure a cisco 2610xm with a nm-hdv and a pri wic for 
callmanager and im not having much luck.  Im working from the following URL

in my callmanager setup i have the mgcp domain name (which does resolve) and 
the following selected - NM-HDV & VWIC-1MFT-T1

on my endpoint identifiers, i have a endpoint-name of S1/DS1-0 at metaswitch_gw
my protocol side is user, selction order is bottom up, sig digits is 10, pri 
protocol type is pri-ni2, linecode is b8zs/esf, clock source is internal, 
everything else is defaults

a show ccm on the router gives
