The GW was not registering with the CM. I fixed that, and all works.

If you aren't seeing any Q931 activity on the gateway, the call isn't
making it there properly.  Check your CCM trace files for things
relating to that gateway.  You should see something to the effect of
"extending call" if I remember my traces correctly.

 
You could also try a test, and use the CMM in H.323 mode and see if the
call makes it that way.

I'm trying to setup my new VGW to connect into a PBX.  I set it up and
dial the route pattern and all I get is a Beep, beep, beep, beep.   Not
a fast busy, not a ring, just a constant beep about every second.  Any
ideas?

I'm also debugging ISDN Q931 and nothing is coming up on the GW.  I am
using CM. 3.3.4, MGCP, IOS 12.3(8)xy (CMM with 6PRI card).

As far as CM is concerned it looks like all is working.  My connections
to the PSTN work just fine.
