[cisco-voip] Unity - delete user

I'm running unity 4.0.2

I have a user that no longer works here and the sysadmin deleted the
users account.  Now I can't delete the user from Unity.  How do I delete
this user?  I've tried multiple times (through web/sa) with no luck.  Is
there a way to manually delete the user?
