I'd suggest opening a TAC case and getting all c:\program files\common 
files\cisco\* files zipped up and attached to the case.

The snippet of logs you included, what file was that from?

Did you install SQL separate from the CM installation?
The CM install will install and configure SQL for you.

When I install the CM4.0 in my server, I found a error code:1603 fatal 
error
during installation. And I look the install log:
