[cisco-voip] file not found after upgrade

my 7912s are still in dissarray. some simply revert back to v5 firmware and some continously reboot.

  After upgrading from 4.12 to 4.13 I reboot my 7940/60 and getting host
  file not found.  I go into the phone configuration click update and
  reset then it finds the file and all is good.  Any way to do this across
  the board?  Or better idea?  I'm open.
