[cisco-voip] Conference to external number

It's based off of the Media Resource Group List you have configured on 
the phones.  Note that unless you have a hardware conference bridge 
defined then if you remote the software cfb you won't be able to 
conference.    You cannot explicitly define which CFB to use for MeetMe 
and Ad Hoc conferences.
