[cisco-voip] Unity not playing menu

I'm using Unity 4.0.5 with Exchange 2003 backend.  I have one account
that is not playing the menu after playing the first message.  After a
couple seconds it hangs up and the operator comes on and says, "if you
would like to make a call, please hang up and try again"  What's up?  Do
I just need to delete and recreate?  All other accounts(~170) seem to be
working.
