we used our in house programmers and told them what we needed and they came up with a custom app for us.  Not web based but it interfaces with our billing system to automagically send bills each month based on total toll charges.  took him about 2.5 hours to do.

I'd also like to hear about such a beast.
 
But I'd also like to hear about people's experience with setting up their own reports using a custom SQL report tool. I've looked at a few out there that seem quite good and allow you to create SQL reports with user input and are web based.
