I can not see the CFA in the CCMAdmin or the CCMUser for that users page.

lmhosts is used for wins resolution, and hosts for dns. I know
Windows will take hosts over dns and I assume lmhosts over any wins
servers configured, if it is enabled in the nic settings.
For the CFA issue if you look at the person's phone in the CCMAdmin
does it show the CFA set? If so, you need to restart the Database
Layer Monitor server on the publisher and then change the CFA on the
phone via the CCMAdmin web page to synch it up.
If the CFA is not present on the CCMAdmin web page but the ccm where
the phone is registered is still forwarding, try the above but if it
does not fix the issue you may need to restart the CCM process on
that node.

I'd first verify hosts and lmhosts entries are added for SINV-IPLA-
CM-01 on the subscriber server.
Are you doing a new install of a subscriber or an upgrade? For a new
install SQL should not already be installed on the subscriber server.

I have tested to connect using the Enterprise Manager, and it works
both with SA and Windows Authentication.
Should I manually configure anything with ODBC?
The SQL version is on both servers, Microsoft SQL Server 2000 -
8.00.2039 (Intel X86) May 3 2005 23:18:38.
