[cisco-voip] translation rule - MGCP

It will work on any call to the destination specified in the 
translation rule.  If your calls come into the gateway as 4-digit 
numbers that match your internal extensions exactly then calls made 
from IP phones may hit the same translation patterns that calls from 
the gateway do.  Constructive use of partitions/CSS can avoid this.
