[cisco-voip] various incoming calls are sent to one ip phone

we have a crazy issue with one of our ip phones 
we are runing cm 4.1.3 and unity 4.0.4  and for some reason various incoming calls get sent to this one particular ip phone (ext 1540) . we think it has to be some kink of DB coruption.  we do not have any issues with anything else.  i call comes into AA unity and caller dial the want party by their ext. and no matter what they put, it will still go to 1540  this is the only phone that is doing this and it has been going on for about 4months.  we deleted the mailbox/subscriber, did not help.  can anybody point me in the right direction
