I've seen many environmental causes of this. If the room itself is hollow
enough (like one wall is against metal siding for a factory), the sound from
the phone actually bounces off the room and back into the phone after the
echo canceller would normally pick it up.
I recommend relatively padded/sound deadened areas, as any tinny room can
cause problems like this.

We get occasional complaints about poor quality and echoes between 2 7940s,
often on the same switch. Comes and goes, but I've heard the problems on my
phone now and then. Not often enough to worry too much about it, but I'd
sure like to know why, especially why echoes. Same software as yours.
