You might check to see if anything else has grabbed the audio hardware.
Even something simply like a copy of Real Player sitting idle in the
background and sometimes fubar wave device registration.

You might also check your JRE version and try upgrading to 1.5 if possible.

Is there a list of supported soundcards/hardware that
IP communicator will work with? 

After selecting the audio devices in audio tuning
wizard, it comes up with window that says "Audio
tuning wizard cannot configure this device to play
sound. Cisco IP Communicator may not function properly
with this device".
