[cisco-voip] MTP errors

As Ryan stated these are usually a problem when a transcoder is being
invoked and not available. If this was during normal call setup then
the call would be disconnected. I have seen many cases where these
errors show up when MoH is being invoked. For example, if the MoH
server is not configured for the G.729 codec but the Region config
would use G.729 between the MoH server and the held device then a
transcoder would be invoked. If none are available you will see this
error but the call won't be disconnected there just wouldn't be MoH.
You can either enable the G.729 codec or provide a transcoder for the
device being held. Detailed CCM traces show this issue too.
