[cisco-voip] AC 'failed to make new call' warning window in CCM 3.3(3)

AC displays that message when it detects a new call attempt originated
from the device and that new call attempt fails.
The call should show up in TCDSrv traces and in the CCM SDL traces. If
you grab CCM SDI and SDL traces, TCDSrv trace, and acclient trace (turn
on through client settings) you should be able to correlate the calls
and see which new call failed.
