[cisco-voip] Route pattern prefixing a "+"

Maybe it's not the best solution but you may try to use Sip Express 
Router (SER) or OpenSER as the SIP backend for the CME and you can 
rewrite the number in the SER.
It's pretty easy but you will need one another server to run SER (it can 
be any unix like station).
