[cisco-voip] call forward not working

When we have run into this problem in the past two things have fixed it.
a) Restart the Database Layer Monitor service on the PUB then the SUB.
Restart TFTP on PUB and then the SUB. Then we usually have to go into
the phone in CallManager and manually take off CFWDAll and restart the
phone. Sometimes it won't even show that CFWDAll is enabled so we
enable it (to voicemail), Update the phone, then disable it, and Update
the phone. This seems to happen with the databases get out of sync even
though DBLHelper shows that everything is OK.
b) Restart of the CallManager servers in the cluster

Call forward is stuck, no one is able to forward or take a forward off
their phones. I checked to see if the phones have reverted over to the
PUB but they have not. Any suggestions?
