[cisco-voip] call forward not working

We've had this same problem since 4.0.2, although its gotten less and less
frequent. I guess there are several defects that could cause it. Still, its
good to know that there is another potential fix. We are still running SR3a
so maybe a minor SR upgrade will help us out.
