Bill,
We did this a couple of months ago and it wasn't too bad.
1) When you create the new voice mail ports for the secondary server
keep the name short. (Don't remember exactly why, but I think it's
something when you run UTIM and do the integration).
For example mine have the name: FOCiscoUM1-VI86 (All I really put in
here was "FO", the rest is generated by the system).
2) Assuming you are on CALL MGR 4.X you will need 4 line groups when all
is said and done if following the Cisco docs.
There are 2 line groups for the answering ports and 2 line groups for
the dial out ports. (1 of each going to their respective servers).
For example our line groups look like this:
MAIN Server ANS Ports 20001-20072
Main Server Dial Out Ports 20073-20096
FO Server ANS Ports 20501-20572
FO Server Dial Out Ports 20573-20596
I am mentioning the line group stuff because I don't recall that being
very clear to us in the Cisco docs. I would suggest opening a TAC case
to help you along the way as we did in case you get stuck.

Looking for any special install notes/gotchas from people who have setup
Unity 4 in a failover situation. We're preparing to do it this weekend.
Engineering group tested it but I just want to see if any of you
encountered anything that strayed from Cisco's docs or your common-sense
expectations.
