FYI,
It looks like you can safely delete the bad/corrupt BAT Phone template .txt files from the \ciscowebs template subfolder fine to get rid of them.
Not sure if there is another fix for the bug CSCee67365 or not. But it seems when the template becomes bad, when you insert phones with the template it has a dialog box with subscript out of range error. However, the validiate phones step in BAT validates susccessfully.

When you say, 'But tried to insert more later, and the insert failed,' do you mean that you tried to insert from the original file, or from a new one?
It seems likely that there was a problem with the original file.
I would create a new one to re-add the ones that failed the first go around.

We created a new phone template in BAT and the phones are inserting fine now, but there are 2-3 dozen pre-existing BAT phone template files that seem to have gone bad as they can't be viewed in BAT tool. Any one have a idea or seen this before? Is it ok to just delete the .txt files under the template folder and recreate them as needed since the BAT tool can't pull them up through the web page to delete them that way.
