That sounds like a power problem with the switches that power the
phones.
We had a similar problem on a 6509 chassis, with about 70 phones
attached to two 48port powered blades.
Here's what happened:
The rated 'wattage' output available on the Cisco 6509 is only available
when running 220v
They are 3000watt power supplies, according to Cisco- that is true--
But they do not provide that full 3000 watts if you're running at 110v.
We only had something like 1100 watts of power available from the power
supply---
the phones needed 1250 - That lack of wattage was enough to destabilize
the phones.
After running into that, I did some research-
Apparently, in the IT industry, it is VERY common to state wattages
(Dell servers, IBM servers, HP switches, Nortel switches)
all state a rated wattage output --- but they only deliver that wattage
at 220volts --- NOT 110v.
James, what you're experiencing sounds very much like a 'low power'
issue--
whatever switches you are using to run the phones may be overloaded for
some reason.
Cisco's 1U Catalyst switches do fine - the 3560 switches are able to
power a phone connected on every port using 110v.
Another possibility is that the power coming into the switch power
supply is low-- if that's the case, the switch isn't going to be able to
provide enough power to run the phones.
Same would be true even if you had bricks connected to strips in that
one wiring closet-- if the voltage is low coming to that wiring closet,
the bricks would all be fighting to get enough current for the phones...

What version of CCM are you running?
What phone load are you using?
What does your basic network look like that this is happening in? L2 STP
Rapid STTP or L3 to the edge?
Every system has some bug or the other but I have worked with CCM since
2.0 and I can tell you that 4.1(x) is way better for which I have not
had many problems at all with.
Just remember there are a lot of moving parts in CCM that all have to
place nicely together and some of the under lying services are all not
written by the same team, that I am aware of.
