[cisco-voip] CVTA and MGCP

I don't know of any specific issues with 4.1(2) and MGCP GWs.  I would 
start by looking at "debug isdn q931" on the GW or looking at the CCM 
traces to see if the call is being rejected by the PSTN.  Compare this 
with a good call.  Most of the problems with CVTA and GWs are due to 
the Bearer Capability presented to the PSTN.
