With VM only the accounts don't have to be local. If they are local
it just means that the Unity server is the only DC and only Exchange
server in the domain. For VMO with failover you still have to have
an off-box AD DC/GC and Exchange server.
Some people even do VM only with their existing AD/Exchange
infrastructure by giving users 2 AD accounts, one for email and one
for vm.
Bottom line is for any subscriber you want to have access via the TUI
there has to be a mailbox and thus a user account, end of story.

local setup, but it would be nice if this would be addressed because
quite frankly UM is not needed for every VM box. If you could have a 100
of these "service" type mailboxes it would be nice. It wasn't a big
deal at first, but we probably have about 12-15 of these accounts now.
We are currently about 15-20% IPT so I see this number rising as IPT
expansion continues....

Remember even with a non UM system you still have an AD Account and
mailbox for each subscriber. The only real difference between
voicemail only and UM is whether the user gets email in the same
mailbox that they get voicemail.
Unity does have a thing called internet subscriber. This is
basically an email address that Unity will forward all voicemails
to. An internet subscriber cannot listen to VMs via the TUI (afaik)
and do not count against the subscriber license count.
