That looks/feels like bug id CSCsc11833.
Do a 'show diag' on your router and look at the PVDM info. The newer PVDM2 module is Fab version 3 with deviation of something other then 0 (83077 in one I have) or fab version 4.

I think this is a bug. I had this same problem on an MGCP gateway. It
appears that some of the PVDM 16 DSP's have an issue. TAC swapped this
out for me for a PVDM 64 and it has occurred since.

I have a 2801 with a DSPVM and a 4 port FXO card configured as an H.323
gateway to cisco callmanager 4.13. Every several days, we get all busy
signals even though all lines are on hook, and the only way I can
correct
the problem is to reset the DSPs. Here is the show log output after
issuing
the commands

Note the error below: "On_hook line reversal detected possibily due to
crossed cable." We are sure that cables and wires are not crossed, is
there
anything else that might cause this error message? My initial thoughts
were
it might be a voltage problem from the fxs ports on the PBX?
