Just in case this ever happens to anyone else in the future..... I
wanted to put it out there for the Google search.

 

Unity 4.0.5 UM VMO Pop up

 

If you have the users having a problem with a Pop up coming up with
enter username / password / domain when trying to play to there phone
here are some steps to fix it.

 

On the Unity server go to administrative tools, component services,
DCOM, AvCsGateway, right click, properties, Security, choose customize
and edit , then add domain users  and give them all permissions and see
if that does it.

 

This fixed it for me.
