The last piece would be to delete the old servers from the
GlobalLocation table in SQL. You'll see three unity servers there. Be
extremely careful while removing the two old servers.

OK. I went through and uninstalled UnityPBX, but my one remaining Unity
server sees both old (dead and gone) unity servers.  How do I remove any
last pieces of old Unity servers?

Before running the Unity Uninstall Tool I'd undo 'Digital Networking',
i.e. set your Dialing Domain to 'None' in the Networking -> Primary
Location on the Unity servers and adjust your Search scopes
appropriately.

As for cleaning up an improperly uninstalled system, you should probably

take a look at the help page for the uninstall tool:

http://www.ciscounitytools.com/App_UninstallUnity30.htm

It lists what the tool does, so you can manually do the pieces that are 
relevant.
