Look at the application event viewer on your failover server. There
will be an event when it is triggered to become active. Most of the
time when the primary doesn't have any issues failover is initiated
because somebody calls one of the failover ports directly or your
primary ports fill up and a call is routed to the failover server.

Have you taken a look at the Failover Monitor on the failover server?
Is the Failback button enabled? If it is then you are in failover.

Our system is programmed so that our primary Unity server is supposed to
be active. Is there any reason why the failover server would be active
all the time? This has been happening for almost a week.
