This is a quite outdated answer but I hope it will help you :)
If you want to add your own URL as the 4th Option in the Directory Menu,
then you have to create a XML (or PHP) file with a code like:

<CiscoIPPhoneMenu>
<Prompt>Select a directory</Prompt>
<MenuItem>
<Name>TUKE Yellow pages</Name>
<URL>http://stargate.cnl.tuke.sk/~jozjan/voip/tlf_zoznam.php</URL>
</MenuItem>
</CiscoIPPhoneMenu>
If you point the Corporate directory URL in your CCM to this file, then your
phones will display as the 4th option the line "TUKE Yellow pages". If a
user selects that option, his/her phone will open the XML page at
http://stargate.cnl.tuke.sk/~jozjan/voip/tlf_zoznam.php.

Thanks everyone for the info - I tried changing the Directory URL in
Enterprise Parameters and it would not work for an address that was not on
the server itself, the same address (on a different server) worked under
Features and Phone Services. Does that sound right?

Options 1-3 (missed, placed, received calls) are built into the phone and
can not be changed.
Option 4 is configured via Enterprise Parameters 'Directory URL'. The
server can also return more than one option and they will show up as Option
5, Option 6, etc.
