Don't have a sample config...

However, you just need to make sure you create two dial-peers for each
outbound destination-pattern, and add a preference statement on each.

Also, a pair of dial-peers on the inbound (for each) with 'incoming
called-number .' and a 'direct-inward-dial' to ensure you get digits and
send them to CCM.

Also, make sure you have a voip dial-peer pointing at each CCM with the
pattern for your DID range, with the best preference to your sub.
