[cisco-voip] problem with 3745 configuration and FXO disconnect

I'm having a really strange problem with my Cisco 3745.

It's a lab machine, setup at home, so using FXO ports.

It's a 3745, Vic2-2FXO card.

AIM-CUE installed

Config is below. Here is the problem:

When a call comes from the outside, caller hears the phone ringing.

Watching the phones inside, the phone rings 1 - 2 times, then stops

Caller continues to hear ringing

Phone inside starts ringing again, 

Phone can be answered either time as it shows an incoming call

First time the caller-id shows the caller's phone number

Second time the caller-id shows the trunk from the ephone-dn

If it goes to voicemail, phone indicates 2 missed calls

I've tried a lot of different configs, but can't figure this one out. Anyone
have similar problems or have suggestions?
