[cisco-voip] CCM 5.11 MOH error -- Unmapped exception

When trying to add a MOH file, the find and list comes up with this.

They had a MP3 file they converted to a WAV file then this happened
when they tried to upload it. The new file shows under MOH audio
sources but saids the file is not there and lists no properties for
the audio source.

Unmapped exception. The entity name must immediately follow the '&' in
the entity reference.
