Believe it or not, it is an IOS bug.

We had the same problem since we upgraded a cluster from 6.0.1.2000-4 to 
6.0.1.2108-1. It is resolved in an interims version (12.4(17-9)T i 
believe) or you wait for 12.4.17T.
