Hello,
Check out the callmanager compatibility matrix, I think it will answer your
version questions.

If you have a CCO account with a valid support contract you should be able
to order the upgrade kit(s) with the product upgrade tool (sorry I dont have
a link, you should be able to find it on CCO)
I believe DMA is what you need to use for upgrading to CCM v5.
