[cisco-voip] CM 4.1(3)SR5/5a

We are running it here in a single node cluster, about 300 phones,
Unity, IPCC Express, MPE, and Informacast and no problems as of yet.  We
did run into a weird "hidden dialog" box in regards to CDR during the
install though (on pubs and subs).  Hitting enter seemed to allow the
installation to continue and there were no errors in the logs, but I
couldn't see what it said (since the install window was always on top
and it was non-movable) which is a bit uncomfortable.
