[cisco-voip] CCM 5.1 - Bulk Admin FAC's doesn't work

I am trying to Bulk Insert some FAC’s and it’s not working.  Here’s what I
get in the Job Scheduler:

INSERT for 240 FORCED AUTHORIZATION CODES could not be processed.

INSERT for 0 FORCED AUTHORIZATION CODES passed.

INSERT for 0 FORCED AUTHORIZATION CODES failed.

Any ideas?  TIA
