[cisco-voip] Unity MWI Request is Pending---Forever

I have seen this problem when changes are made with the Exchange server.
After changes, Unity needs to log out of the Exchange mailboxes.  The
easiest way to do this is reboot Unity.

The phone and lines are in the same partition as working users...  I
recently tore down the unity account and rebuilt it and now it works...
Not sure what exactly happened.

Is he on a separate partition then the other 'working' users ?
 
MWI will use CSS to get to a phone.

I have one user whom I cannot get the MWI light to fire when they have
new messages in thier inbox.  Looking through unity web interface, I see
under subscriber -> messages an MWI extensions portion where X is in the
extension box and the MWI request is pending.  It will be that way until
the message is read from their outlook client.

Any Ideas of what could cause this???
