[cisco-voip] Outbound call routing

If you're using MGCP it's all up to callmanager. Using the 8.@ pattern
to match and discarding predot, prepending the access code (9200?)
should do the trick. Is it the lack of a pause between access code and
dial string that's tripping up the PBX?
