maybe you could start by debugging the communication between the GW
and the PBX, so you can see if the PBX is actually rejecting the call
setup.

if its a analog interface (fxs, fxo, did, e&m) or a digital interface
configured for CAS "debug vpm signal" should produce some readable
output.

if you are using a ISDN connection (BRI, PRI) than "debug isdn q931"
is a good place to start... but if it is really a ISDN connection than
the pause between access code an the other digits should not be an
issue...
