[cisco-voip] CUCM 5.1 fixed-source MOH

Has anyone successfully implemented the USB adapter for fixed-source MOH
on Callmanager 5.x? We are struggling getting ours working, and there's
not much experience within Cisco on it. We have the required adapter,
the configs are apparently correct, but the device will not start
according to the traces.
