Blank translation pattern pointing to 5929?

This is totally weird.  Customer is reporting that extension 5929 gets
calls that are not even trying to reach the caller.  weirder yet
someone on an IP Phone picked up his phone to dial and it
automatically dialed this 5929 number (was already ringing).

It seems to be all IP Phone based.  I'm at a loss.  I have no PLAR
lines setup so how does it do this?

I need to pull some CDR's and take a look, but I'm just trying to see
if anyone has seen anything like this.
