[cisco-voip] Could not create CTI Route Point and corresponding Line on CallManager

I ran into this issue once before. The problem was that the credentials
being provided to the IPCC Express server to log onto the Callmanager server
were incorrect.
Verify that the user id and password you're using is correct, and you might
also take a look at the application event logs on both servers.
