[cisco-voip] UPGRADE CCM 4.1(3)es126 to 4.1(3)SR6a

SR6a is based off of ES126 but has fixes that are also scattered from  
ES126 through ES133.  This means that you can install SR6a over top  
of any ES <= 126.  Anything higher than 126 but less than 134 you  
should uninstall before installing SR6a and anything >= 134 has more  
fixes than SR6a.
