[cisco-voip] Call Pickup Groups Not Working

I have a client on 4.2(3)sr2b. They have crazy problems with Call Pickup Groups. Phones that are set in the right groups will all of a sudden be not able to pickup calls from phones in the same group. Removing/readding lines to pickup groups never works, rebooting the CM servers fixes the problem but is never permanent the groups just seem to randomly break.
Has anyone seen this and have any ideas about cause and solution? This was happening when were still on CM 3.3 as well
