Alternate extensions makes sense (press the messages button on the phone) so 4 digit extensions need to be entered as alternate extensions.
But why alternate MWIs?

You might also need alternate extensions and alternate MWIs in order to get things working right. That's if you also use Unity as your AA.

1. Create a Voice Mail Profile with a Voicemail Box Mask that prepends the digits required.
2. Apply the Voice Mail Profile to the respective directory numbers
3. Change the Mailbox ID’s to reflect the required digits

Subject: [cisco-voip] FW: Envelope on Unity Voicemail
Situation: We have 4 digit dialing on our campuses so programming on the UCM DN Configuration is 4 digits.
Problem: We now have a total of 3 NXX’s. When someone from within our organization leaves a voicemail message only the 4 digits are identified on the envelope of the message. If someone is retrieving their voicemail messages from home and wants to return a phone call the challenge is guessing at the NXX.
Question: Is there a work around so all 7 if not 10 digits are included in the message envelope?
