The issue usually will surround someone changing passwords on system
accounts in active directory. (or expired passwords)
Check all AD unity system accounts and make sure they are correct.

Subject: [cisco-voip] Unity will not restart
I have an issue with a Unity server, to begin there was issues with not
being able to get to mailboxes, so I decided to stop and restart the unity
service from the system tray, I was able to stop the service fine but it
would not restart. I have rebotted the server adn get the same issue. the
exchange is a separate box and this services Unity only so I have restarted
this server and then restarted the Unity box again and I am still unable to
get the Unity service started. Can anyone provide any help with
triubleshooting this issue.
