I got this working with some offline help from a list member.

Had to use application dial rules,  any +'s, spaces, dashes, ('s in
the telephone number field AC strips out and just uses the numbers.
You can see this if you pull up the dial window in AC then click on a
person in the directory with a telephone number with characters and
spaces in it, etc.  Then you base the application dial rule off of the
stripped down number AC is using.  You need to restart AC for it to
see the rules, then when you click on the person in the directory the
dialed number will be whatever the dial rule is set to do.
