On this, I usually have to do the Update Users to associate the device
or profile then have to run it again to set the Primary Extension. The
Primary Extension does not set until a device of some sort (or device
profile) is associated.

Were you able to specify the primary extension during the update of the
users.
we tried, however it does not set the primary extension in cm 6.1.2.
I have opened a tac case for that, and we are still trying, to get it
done through BAT.

And on the BAT file just fill out username and Controlled
Device/Profile 1 (you will need to change Number of Controlled
Devices/Profiles from 0 to 1 first) and then BAT it in...
