if you use Unity as your auto-attendant, you will not be able to get "double" rings for off-net calls. Unity acts like a skinny phone and is transferring the call to your extension.
if you use callmanager scripting for auto-attendant, it uses CTI so you will get double ring. similarly with CUE.

Subject: [cisco-voip] Distinctive Ringlist for 6.1 Internal and externalcalls
I'm trying to understand how the distinctive ringlist on 6.1 works, I have checked and read the archives about distinctive ringlist and nothing seems to be matching up, one post I read advises that you have to set the service parameter H323NetworkLocationOffNet to True. However i cannot find this on 6.1, another post says it is already switched on and is determined from the onnet / offnet settings on the gateway.
But how do you actually select the ringtone for internal or external calls.
Has anyone successfully configured this on 6.1 and can provide some pointers?
