[cisco-voip] Voicemail recording fails on Cisco Unity - Help needed

Greetings,
We are facing a VoiceMail recording problem with Unity Connection
version: Unity version = 1.2(1.0) Patches installed: (none)
I am unable to leave a voicemail message from our wireless phone (SIP) and also with the pjsip (open source pjsip.org) soft phone.
The logs on the Unity server indicate that this is a recording problem.
excerpt:

Attached are the log files (zipped) collected using Unity Diagnostics. Any help to troubleshoot/direction is appreciated
